Care that doesn't stop at the referral

Addiction, mental health, physical health and rehabilitation are treated as separate services almost everywhere. For the person receiving them, they are not separate at all.

Oblix supports all four as one connected cycle — or each on its own, if that's how your practice works.

Why the cycle matters

Co-occurring need is the norm, not the exception. Someone in detox may need psychiatric support. Someone in recovery may be managing physical consequences that began years earlier. Someone finishing an in-patient programme needs the aftercare to know what happened during it.

When those sit in separate systems, the connective work falls to the patient — or it doesn't happen.

How it holds together

One relationship, not four episodes.

A patient moving between pathways stays the same patient, with the same history available to whoever is caring for them at that point.

Context travels.

What was shared during pre-care is available in treatment. What happened in treatment informs aftercare.

Practitioners stay in their lane.

Working across pathways doesn't mean working outside your specialism. It means the consultant you refer to already understands the picture.

Or run them independently

Not every practice needs all four. Each pathway works completely on its own, with its own stages, roles and settings.

Start with one. Add another when it makes sense. The platform doesn't require you to adopt a model you don't practise.

Pathways we haven't built yet

The four pathways are configurations of the same underlying model — stages, roles, consultations, records and reviews.

Other pathways can be designed the same way, for any specialty with the same shape: a journey with phases, several practitioners, and continuity that matters between appointments.
